INSURANCE

Fairfax Physical Therapy provides you with an estimate of your portion of the charges and asks you to pay that amount at each visit.  In some instances it is the policy of the insurance company to send reimbursement checks directly to the patients instead of to the clinic, in which case you pay in-full and keep the insurance checks.

As a courtesy, we bill your insurance company for you.  Out-of-network physical therapy benefits generally cover between 50-80% of the physical therapy charges, depending on your coverage. 

It is your responsibility to keep in contact with our office staff and your insurance company to ensure all authorizations, referrals and payments are in order.  Once Fairfax Physical Therapy receives payment from your insurance company, you will be responsible for any amount they have not paid.  If you have any questions about your insurance coverage, please call your insurance company.

We are preferred providers for:

Medicare

Tricare

Veterans Affairs (VA)

and BlueCross BlueShield and all other CareFirst Plans

We are out-of-network with all other insurance companies, but many of those do provide out-of-network benefits, and we submit the claims for you so there is no need for you to deal with any additional paperwork.  Examples of insurance plans with out-of-network benefits include:
